COVID19 PFIZER VACCINE

Pzifer COVID 19 vaccine is now available at our office, for all patients 12 years and older, regardless of insurance. We are excited to be aboard as an official COVID vaccination site (for our established patients and families).

STEPS:

1) Please call 520.298.3383 to schedule your child's COVID19 vaccine appointment.

2) For the appointment, your child needs to be accompanied by a parent or legal guardian.

3) At check-in, you and your child will sign a consent and be given the V-Safe Information sheet and the Pfizer COVID-19 Fact Sheet (see pdfs below)

4) After the vaccine, you and your child will need to sit in the exam or waiting room, for observation.
